“Missed canal during endodontic treatment is the most common cause of failure in root canal treatment. Mandibular premolars present anatomic challenges due to extreme variation in their root canal morphology. Accordingly, proper knowledge of all root canal system variables, radiographic imaging with careful interpretation, proper selection of armamentarium and precise maneuverings during access cavity preparation can provide safe solution to achieve access cavity designs providing exposure of all root canal system contents. The current presentation will provide clinical cases during my practice, showing mandibular premolars with morphological variation and my interventions to resolve this challenge and the academic background available in literature relevant to this issue.
Conclusions: Precise knowledge of root canal morphology and using appropriate techniques provide predictable way to manage mandibular premolars with morphological variations.”
